Practice Pharmacist – Primary Care (South West Fife)

About the role

The NHS Fife Primary Care Pharmacy Team is seeking a motivated Practice Pharmacist to join its South West Fife service. You will work closely with GP practices to deliver high‑impact pharmaceutical care, including medication reviews, prescribing support and independent prescribing.

Key responsibilities

• Conduct structured medication and polypharmacy reviews in GP practices.
• Provide medicines reconciliation and support acute and repeat prescribing.
• Utilise independent prescribing authority to optimise patient care.
• Offer specialist medicines advice to clinicians and patients.
• Participate in quality‑improvement projects, audits and service development.
• Collaborate across primary care and community pharmacy interfaces.

Required profile

• Registered pharmacist with eligibility to prescribe independently.
• Enthusiastic about delivering patient‑centred care in a multidisciplinary team.
• Willingness to develop clinical skills through mentorship and CPD.
• Flexibility to work full‑time (37 h) or part‑time as needed.
