Practice Pharmacist – Primary Care (North East Fife)

About the role

We are looking for a motivated Practice Pharmacist to join NHS Fife’s Primary Care Pharmacy Team in North East Fife. You will work closely with GP practices to deliver high‑impact pharmaceutical care, including medication reviews, medicines reconciliation and independent prescribing.

Key responsibilities

• Provide high‑quality pharmaceutical care to patients in GP practices and community settings.
• Conduct structured medication reviews, polypharmacy reviews and medicines reconciliation.
• Undertake independent prescribing and support acute and repeat prescribing.
• Offer specialist medicines advice to clinicians and patients.
• Support safe, effective and cost‑effective prescribing.
• Participate in quality‑improvement projects, audits, guideline development and service innovation.
• Collaborate across primary care and community pharmacy interfaces.

Required profile

• Registered pharmacist with a commitment to patient‑centred care.
• Clinically capable and enthusiastic about primary‑care pharmacy.
• Ability to work within multidisciplinary teams and communicate effectively with clinicians.
• Willingness to engage in continuous professional development.
