Relief Programme Facilitator – Fixed Term (6 months)

About the role

Join an independent voluntary organisation with over 70 years of experience supporting people with disabilities and their families. As a Relief Programme Facilitator you will promote independent living and community integration by delivering educational, developmental, recreational and occupational programmes tailored to each service user’s needs.

Key responsibilities

• Develop and deliver person‑centred programmes identified through Person‑Centred Planning.
• Act as a key worker, facilitating the planning process and maintaining accurate records.
• Identify individual goals and required supports, documenting progress as required.
• Support individuals and groups in accessing community activities and resources.
• Observe all RehabCare and Rehab Group policies, rules and procedures.

Required profile

• Minimum QQI Level 5 in Social Care or equivalent qualification.
• Good knowledge of New Directions and community‑based programmes.
• Understanding of intellectual disabilities and mental health issues.
• At least one year of experience in a relevant setting, including facilitating person‑centred plans.
