Senior Pharmacist – Health of Older People

About the role

The Senior Clinical Pharmacist – Health of Older People will lead pharmacy services for the Health of Older People (HOP) program across inpatient, home‑based and community settings. This role is based at Caulfield Hospital and focuses on medication management for patients with chronic or complex age‑related conditions.

Key responsibilities

• Develop and deliver pharmacy services within HOP inpatient units.
• Coordinate medication safety and optimisation for older patients across hospital, community and home settings.
• Participate in multidisciplinary ward rounds and collaborate closely with doctors, nurses and allied health professionals.
• Mentor junior pharmacists and pharmacy students, supporting their clinical education.
• Lead research and quality‑improvement projects related to geriatric pharmacotherapy.
• Ensure compliance with hospital policies, accreditation standards and best practice guidelines.

Required profile

• Registered pharmacist with extensive clinical experience.
• Demonstrated leadership in a hospital or aged‑care pharmacy environment.
• Experience managing chronic or complex conditions common in older adults (e.g., dementia, falls, continence, bone health).
• Strong commitment to interdisciplinary teamwork and continuous professional development.

Required skills

• Clinical pharmacy practice.
• Geriatric pharmacotherapy and medication safety.
• Use of pharmacy information systems and electronic medication records.
• Research methodology and quality‑improvement techniques.
