Nurse Practitioner / Trainee – Hospital at Home (Annex 21)

About the role

An exciting opportunity has arisen within the Hospital at Home service for a motivated Nurse Practitioner or Trainee Nurse Practitioner to join the East Team at Adamson Hospital. The role supports safe, effective, person‑centred care for older adults and frail patients in primary‑care and community settings.

Key responsibilities

• Deliver urgent and unscheduled care to older adults, including acutely unwell, palliative, and frail patients.
• Provide clinical assessment, treatment and management for nursing and care‑home residents.
• Travel across Fife promptly to attend to acutely unwell patients.
• Work a 7‑day rota covering 08:00 – 22:00, collaborating with the wider Health and Social Care Team.
• Maintain compliance with the Protection of Vulnerable Groups (PVG) Scheme.

Required profile

• Current NHS Fife employee (internal posting).
• Completion of the Clinical Assessment and Decision‑Making module.
• Preferably hold Level 11 Non‑Medical Prescribing qualification.
• Excellent communication and ability to work autonomously.
• Eligibility to work in the United Kingdom.
