RN Wound Care Nurse – Hybrid

About the role

PACS is looking for a compassionate Registered Nurse (RN) to provide specialized wound care across two skilled‑nursing facilities in Westerville and Dublin, Ohio. The hybrid position requires travel between sites to ensure consistent, high‑quality wound management for residents.

Key responsibilities

• Assess, treat, monitor, and document wounds and pressure injuries.
• Communicate care plans, progress, and concerns with residents, families, physicians, and interdisciplinary team members.
• Administer prescribed medications and treatments while preserving resident dignity.
• Update and maintain accurate nursing notes and care plans.
• Notify physicians promptly of any lack of healing, infection signs, or adverse changes.
• Assist physicians during examinations and coordinate wound‑care resources such as specialty mattresses and nutritional supplements.

Required profile

• Current RN licensure in Ohio.
• Experience in wound care, pressure‑injury management, and skilled‑nursing environments.
• Strong communication and customer‑service skills for interacting with residents and families.
• Ability to travel between the two facilities as needed.
