Advanced Specialist Pharmacist – Pharmacotherapy

About the role

This permanent, full‑time position is based at Galway University Hospitals and is aimed at an experienced pharmacist who will act as an Advanced Specialist in Pharmacotherapy. The role involves leading complex medication reviews, supporting multidisciplinary teams and driving high‑quality clinical decision‑making across the hospital.

Key responsibilities

• Conduct advanced pharmacotherapy assessments and complex medication reviews for in‑patients.
• Collaborate with physicians, nurses and allied health professionals to optimise drug therapy.
• Provide expert advice on chronic disease management, polypharmacy and high‑risk medicines.
• Develop and implement therapeutic guidelines and protocols.
• Mentor junior pharmacy staff and contribute to continuous professional development programmes.

Required profile

• Post‑qualification experience in pharmacotherapy with a proven record of advanced clinical decision‑making.
• Demonstrated ability to work within multidisciplinary teams.
• Experience in specialist areas such as chronic disease management, polypharmacy or high‑risk medicines is desirable.
• Registration with the Pharmaceutical Society of Ireland.
