Nurse Practitioner / Trainee Nurse Practitioner – Hospital at Home

About the role

An opportunity has arisen within the Hospital at Home service for a motivated Nurse Practitioner or Trainee Nurse Practitioner to join the East Team at Adamson Hospital. The role supports safe, effective, person‑centred care for older adults in primary‑care settings, focusing on urgent and unscheduled care.

Key responsibilities

• Deliver urgent and unscheduled care to older adult patients, including acutely unwell, palliative and frail individuals.
• Provide complex treatment and management for nursing and care‑home residents.
• Work autonomously, using clinical judgement and decision‑making to assess and treat patients.
• Travel across Fife to attend to acutely unwell patients, covering a 7‑day roster (08:00‑22:00).
• Collaborate with the wider Health and Social Care Team and adhere to NHS policies.

Required profile

• Current NHS Fife employee (internal posting).
• Hold the Clinical Assessment and Decision Making module; preferably completed Level 11 Non‑Medical Prescribing.
• Excellent communication skills and ability to work independently.
• Willingness to become a member of the Protection of Vulnerable Groups (PVG) Scheme.
