About the role

The Health Service Executive (HSE) is seeking an experienced pharmacist to join Galway University Hospitals as an Advanced Specialist in Pharmacotherapy. This permanent, full‑time position focuses on delivering high‑level clinical decision‑making, complex medication reviews, and multidisciplinary collaboration.

Key responsibilities

• Provide advanced clinical decision support for medication therapy across acute and chronic settings.
• Conduct comprehensive medication reviews, particularly for patients with polypharmacy or high‑risk medicines.
• Collaborate with physicians, nurses, and allied health professionals to optimise therapeutic outcomes.
• Lead specialist initiatives in chronic disease management, high‑risk therapeutics, and medication safety.
• Mentor junior pharmacy staff and contribute to continuous professional development programmes.

Required profile

• Post‑qualification experience in pharmacotherapy with demonstrated depth and breadth.
• Proven ability to make advanced clinical decisions and manage complex medication regimens.
• Experience in specialist areas such as chronic disease management, polypharmacy, or high‑risk medicines is desirable.
• Strong commitment to multidisciplinary teamwork and patient‑centred care.
