About the role
The Community Nurse will join the Horizons Community team to deliver responsive, person‑centred care for the People We Support (PWS). Working 37.5 hours per week, the role focuses on meeting evolving health needs and promoting wellbeing across a range of community settings.
Key responsibilities
- Provide compassionate, person‑centred nursing care to PWS.
- Respond to changing health needs and deliver innovative, sustainable care solutions.
- Promote health and wellbeing across various community settings.
- Collaborate with the Community Nurse Manager and multidisciplinary team.
Required profile
- Level 8 nursing qualification and registration with the Nursing and Midwifery Board of Ireland.
- Valid work permission for Ireland.
- Strong interpersonal and communication skills.
- Flexibility to meet the changing needs of service users.
- Experience with intellectual disabilities or autism is an advantage.
- Full clean driving licence.
- Empathetic, patient, and passionate about helping others.
