Project Worker

About the role

The Project Worker will support the ModelHomecare programme, a collaborative initiative aimed at helping mental health service users maintain secure tenancies and live independently. Working with the Good Shepherd Centre, HSE Mental Health Services and Kilkenny County Council, the role focuses on delivering high‑level homecare support to individuals facing self‑neglect, hoarding or incapacity challenges.

Key responsibilities

• Provide personalised homecare assistance to tenants at risk of losing their accommodation.
• Collaborate with health professionals, local authority representatives and housing partners to develop and monitor recovery‑oriented tenancy plans.
• Assist clients in maintaining their homes to an adequate standard, addressing issues such as self‑neglect and hoarding.
• Facilitate communication between service users, the Good Shepherd Centre and HSE mental health teams.
• Track progress and report outcomes to funding bodies and project partners.

Required profile

• Experience working with vulnerable adults or people with mental health needs.
• Understanding of housing support and tenancy sustainment processes.
• Ability to work collaboratively across health, social and local authority sectors.
• Commitment to promoting independent living and recovery.
